Annual Health and Medical Record
The BSA's Annual Health and Medical Record (AHMR) is required for each participant, which includes parents/guardians and siblings.
BSA regulations prevent Pack 32 from storing electronic copies of this form. If you understand the inherent insecurity in emailing and cloud storage, we can accept them via email. However, if your recipient is unable to print a physical copy, he is not allowed to forward it and you've reached a dead end. Therefore, we prefer that you submit paper copies to your Den Leader, Cubmaster, Committee Chair or designated Committee Member.
It's helpful if you download the PDF and use your computer to type the information. Legibility is very important when dealing with the names of medications or in an emergency, and could mean the difference between life and death. As a bonus, if you keep the files, you'll have the form filled out and ready for updates in subsequent years.
Completed records must include a copy of your proof of insurance.
